Background:To probe the diagnostic value of combining serum tumor markers - Cancer Antigen 19-9 (CA19-9), carbohydrate antigen 125 (CA125), carcinoembryonic antigen (CEA), and Ferritin - with contrast-enhanced computed tomography (CT) for cholangiocarcinoma (CCA). Materials and Methods: A total of 114 patients from Anhui No.2 Provincial People's Hospital from February 2020 to October 2025 were divided into a cholangiocarcinoma group (n=60) and a benign biliary lesion group (n=54). Serum levels of CA19-9, CA125, CEA, alpha-fetoprotein (AFP), and Ferritin were measured in both groups. CT imaging features of patients in both groups were analyzed. Furthermore, the diagnostic value of the serum markers alone, contrast-enhanced CT alone, and their combination was assessed. Results: Serum concentrations of CA19-9, CA125, CEA, and Ferritin were markedly enhanced in the cholangiocarcinoma group compared to the benign group. Cholangiocarcinoma typically presented as circular high-density shadows within the common bile duct or hepatic duct, accompanied by duct wall infiltration, invasion of the common bile duct, hepatic lobe atrophy, and lymph node enlargement. In contrast, benign lesions primarily appeared as circular high-density shadows with minimal duct wall invasion or lymphadenopathy. The combined approach demonstrated superior sensitivity, specificity, and accuracy for identifying cholangiocarcinoma compared to any single method. The area under the curve (AUC) (0.922) for the combined model was higher than the AUC of 0.865 for the serum marker alone. Conclusion: The integration of serum CA19-9, CA125, CEA, and Ferritin with contrast-enhanced CT findings shows promising diagnostic efficacy for cholangiocarcinoma and offers significant insights for evaluating tumor invasiveness.
